Please can you let us know your menu choices for you and your guests by completing the form below.

You will need to fill out a separate form for each person for whom you have purchased a ticket.

So, if you have bought two tickets for yourself and your partner/friend, fill out your own details first and hit the submit button.

Once you have submitted, the form fields will become empty again and you will then be able to complete the form for your partner/friend(s) and hit the submit button again - just to re-emphasise, one form fill per person attending.

It is also important we know who the lead person in the group is, to help us with our seating plan.
